Exercise capacity as a predictor of coronary microvascular dysfunction

چکیده
Abstract Introduction Exercise capacity (EC) is an important prognostic test in patients with coronary artery disease (CAD). Positron-emission tomography (PET) risk-stratifying tool to tailor management of disease. Purpose We aimed assess EC predicting positron emission myocardial perfusion imaging (MPI) derived flow reserve (CFR). Methods Consecutive who had clinically indicated PET MPI and were included. was defined based on peak metabolic equivalents (METS) achieved during exercise stress test. Myocardial blood (MBF) ml/gm/min obtained from dynamic images at rest hyperemia. The (CFR) calculated as the ratio MBF left ventricle. Results study population consisted 511 underwent testing (mean age 56 (±11), 71% male). Cardiovascular risk factors prevalent (75% hypertensive, 82% dyslipidemic, 43% diabetics, 27% known CAD). Peak METS >6 81% patients, CFR <2 present 27%. In multivariable logistic regression models adjusted for age, sex, cardiovascular (hypertension, diabetes, dyslipidemia) medication use (angiotensin converting enzyme inhibitor/angiotensin receptor blocker, statins, beta-blockers), a statistically significant lower odds microvascular dysfunction (CFR <2: OR 0.43, 95% CI 0.26–0.71, p=0.001). Assessment discrimination showed model incremental improvement Harrell's C statistic over clinical variables (0.65 vs 0.69, p=0.02). Conclusion Our findings good predictor health above variables. Further studies are needed interplay between CFR. Funding Acknowledgement Type funding sources: None.

erectile dysfunction as a predictor of early stage of coronary artery disease
background: diagnosis of coronary artery disease (cad) in early stages is vital in decreasing mortality by reducing the risk factors. the aim of this study was to investigate the association between erectile dysfunction (ed) and cad. methods: a total of 200 patients were divided into four groups according to their angiography results: group 1 (g1, n = 59): patients with one-vessel disease (1-vd...
متن کاملPharmacotherapy for coronary microvascular dysfunction.
Coronary microvascular dysfunction (CMD) has been increasingly recognized as an important cardiac condition that can cause signs and symptoms of myocardial ischaemia in various clinical settings. The dysfunction is located on the level of the coronary microcirculation with a vessel diameter of <500 µm and can be characterized by structural as well as functional vascular alterations. The underly...
متن کاملCoronary microvascular dysfunction: an update.
Many patients undergoing coronary angiography because of chest pain syndromes, believed to be indicative of obstructive atherosclerosis of the epicardial coronary arteries, are found to have normal angiograms. In the past two decades, a number of studies have reported that abnormalities in the function and structure of the coronary microcirculation may occur in patients without obstructive athe...
متن کاملCoronary microvascular endothelial dysfunction is an independent predictor of development of osteoporosis in postmenopausal women
BACKGROUND A growing body of evidence links coronary artery atherosclerosis and calcification to osteoporosis in women. The endothelium plays a critical role in maintaining vascular integrity and may play a role in bone metabolism. We aimed to determine whether early coronary atherosclerosis, as detected by coronary microvascular endothelial dysfunction (CMED), predicts the development of osteo...
متن کاملRelation of microvascular dysfunction to exercise capacity and symptoms in patients with severe aortic stenosis
Background The management of asymptomatic patients with severe AS remains controversial and clinical practice varies. Echocardiographic measures of severity do not discriminate between symptomatic status or predict exercise capacity. The purpose of this study was to investigate the mechanisms contributing to symptom generation and exercise intolerance. This needs to be fully understood to optim...
